Spontan Subaraknoid Kanamada Klinik Sonuçlarla İlişkili Faktörler

Amaç:

Bu çalışmada, anevrizmal subaraknoid kanamalı (SAK) hastalarda prognozu etkileyen faktörlerin değerlendirilmesi amaçlanmaktadır.

Yöntemler:

Bu çalışmada 2016-2020 yılları arasında SAK tanısı ile tedavi edilen hastaların hasta dosyaları ve taburculuk sonrası 6. ay poliklinik kayıtları geriye dönük olarak incelendi. Kliniğimizde endovasküler veya mikrocerrahi tekniklerle tedavi edilen 44 anevrizmal SAK’li hastanın taburculuk sonrası klinik durumu, hayata adaptasyonu ve maluliyet durumu Glasgow Outcome Scale-Extended (GOSE) ile değerlendirildi. Hastanın yaşı, cinsiyeti, tedavi zamanlaması, anevrizmanın yeri, Dünya Nörolojik Cerrahlar Federasyonu (WFNS) skalası, Hunt-Hess skalası (HHS), Fisher skalası ve sonrasında GOSE skorunu etkileyebilecek preoperatif değerlendirmeler arasındaki ilişki incelendi.

Bulgular:

Kriterleri karşılayan 44 hasta çalışmaya dahil edildi. Hastaların yaş ortalaması 49,1±10,3 (minimum-maksimum: 29-77) idi. Bunların %54,5’i kadın, %45,5’i erkekti. Hastaların %54,5’inde komorbidite mevcuttu. En sık görülen anevrizma %27,3 oranı ile orta serebral arter anevrizması idi. Anevrizmal SAH’lerin %45,5’i mikrocerrahi klipsleme ile ve %27,3’ü endovasküler yolla tedavi edildi. GOSE skoru ile tedavinin zamanlaması (p=0,014), WFNS (p=0,002) ve HHS skorları (p<0,001) arasında istatistiksel olarak anlamlı bir ilişki bulunurken, hasta yaşı (p=0,47) ve Fisher skalası skoru (p=0,465) ile istatistiksel anlamlı ilişki bulunmadı.

Sonuç:

Çalışmamızda serebral anevrizmatik SAK’de erken cerrahi günü, WFNS ve HHS’nin klinik sonuca etkileri gösterilmiştir.
